Flex Community Resource Coord (Full Time)

This position will be supporting our Rhinelander office and members with a 2 hour driving distance.

Under the direction of the Member Support Manager, provides long-term care Member Support coordination and consultative services to individuals who are members. Services include assessment, care plan development, and Member Support coordination provided under the Wisconsin Family Care Program.
This position is a Flex position within Inclusa, Inc.. Support Our Staff (S.O.S.) Program. Incumbent will have a reduced caseload of assigned members, but will primarily assist on other staff's caseloads as needs dictate.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Provides comprehensive needs assessment.
Develops individualized member-centered plan.
Provides ongoing supervision, monitoring, and follow-along to each assigned case.
Provides all documentation of activities, as required by funding source.
Maintains accurate and up-to-date case files.
Responds to emergent situations.
Coordinates with other community service providers, as necessary.
Acts as a client advocate, as needed.
Provides individual and family consultative services, as necessary.
Provides information and referral services, as necessary.
RequirementsQualified applicants must have a four-year degree in the human services field or a four-year degree in any field, with three years of related experience. Must have a valid driver's license.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
